This page is for CWR’s wanting preapproval for CE not indicated on the Approved CE list on the CWR website. Do not complete this form if you are submitting CE that is on the Approved CE list. To submit CE from the approved list simply upload proof of completion when you complete your renewal application.
The CWR Renewal Form application must also be submitted in order to renew your certification. Payment information is submitted using Renewal form. Confirmation and further instructions will be sent within 3-5 business days once your application has been processed. CE can be sent for approval prior to renewal, but renewal status will not be reviewed until the CWR Renewal form is submitted.
